Step 1: Are you eligible?
My Health Online is available to existing patients, ages 18 and older*, who receive care from one of our affiliate medical foundations or participating community physicians. In order to successfully enroll in My Health Online, you must be an existing Sutter Health patient and have scheduled an appointment or seen a provider. If you are a brand new patient, you may be able to enroll at your first office visit. To find out whether your current doctor offers My Health Online, use our Find a Doctor search and type in your doctor's name. The doctor profile page will show if your doctor offers My Health Online. Once you’ve confirmed that your doctor offers these services, you're ready to sign up for My Health Online.
*Patients 12-17 are also eligible to have a My Health Online for Teens account with a parent or guardian's permission. Ask your child's doctor about getting access.
Step 2: How to enroll
Enroll online and we'll email your Access Code: Sign
- Call our Patient Service Center at 1-866-978-8837 to have an Access Code generated and sent via SMS Text Message to your mobile phone (standard messaging fees apply).
- Mail or fax your enrollment form: You can download the enrollment form here or on the website and send it via mail or fax to our Patient Services Contact Center. Once your information has been verified, an access code will be mailed to your home address. Please allow up to one week for processing.
- Enroll at your doctor's office: You can also enroll at your next office visit. Tell the front desk staff or your medical provider that you would like to enroll in My Health Online. You will receive an activation code to complete the enrollment process.
Step 3: Activate your account
Once you have received your access code, you can activate your account, set your Login ID and password, and begin using My Health Online.
